The form below should be utilized to submit an appeal and upload documentation that falls under the categories of:

• academic dismissal
• programmatic dismissal
• grade appeal
• graduation deficiency
• time limit 

Appeals falling under the category of 鈥渓ate registration,鈥� 鈥渓ate/retroactive withdrawal鈥� or 鈥渞epeating a course鈥� must be submitted to the Academic Standards area of the Registrar鈥檚 Office.
